As a resident of Colorado, you should take the following steps to ensure your decision to become a donor is carried out:

• Sign up online in the Colorado Organ and Tissue Donor Registry at www.ColoradoDonorRegistry.org

• Indicate your wishes on your driver’s license or state ID when you apply for or renew it
