REGISTER TO BECOME A DONOR

As a resident of Florida, you should take the following steps to ensure your decision to become a donor is carried out:

• Go online to view and and printout an organ and tissue donation brochure with registration form for the State of FLorida Organ Donor Registry at http://www.fdhc.state.fl.us/MCHQ/Health_Facility_Regulation/Organ_Donors/index.shtml. The form must be signed and witnessed with two signatures before it is returned to your local Florida Department of Motor Vehicle office or mailed to the Tallahassee address on the brochure.

• Indicate your wishes on your driver’s license or state ID when you apply for or renew it.
