As a resident of Indiana, you should take the following steps to ensure your decision to become a donor is carried out:

• Sign up online in the Indiana Donor Registry for organs/tissue/eyes at http://www.donatelifeindiana.org.

• Indicate your wishes on your driver’s license or state ID when you apply for or renew it
