REGISTER TO BECOME A DONOR

As a resident of Missouri, you should take the following steps to ensure your decision to become a donor is carried out::

• Sign up for the Missouri Organ Donor Registry at http://www.dhss.state.mo.us/OrganDonor/ by downloading the available form.

• Sign up for the Missouri Organ Donor Registry through Department of Revenue when you apply for or renew your driver's license or non-driver's identification card.
